Job Title: Case Manager – (Direct Care) Day Habilitation

  • A copy of a Covid-19 vaccination of two shots will be required
  • New hires who have not received the booster shot, will agree to a targeted date for the booster shot, based on the date of their last vaccine.
  • BA/BS in Human Services / related field or substitute 10 years of related experience in with persons with ID/ DD or human services.
  • Effective oral and written communication skills – please note that as a part of the interview process, all candidates will be completing a 1-page writing sample.

Hours: 40 per week

Annual Rate: $43,620

Hourly Rate: $20.97

SUMMARY:

Lifeworks, Inc is a distinguished human services agency working with individuals who are intellectually challenged and developmentally delayed in a variety of community settings.

Work as a CASE MANAGER (DIRECT CARE) – Develop goals and objectives that each individual in the classroom can work on as a goal. These goals and objectives focus on specific need areas that are identified by each participant’s interdisciplinary team and are documented in each individual’s Day Habilitation Service Plan.

Individuals at Westwood Day Habilitation range from people who require total assistance with all aspects of ADLs, such as requiring overhead lifts and a changing table to complete their toileting needs, to individuals who just need to be shadowed when completing ADLs to ensure that they complete all necessary steps.

Responsibilities:

  • Writing and implementing Individual Support Plan (ISP) as well as completing all documentation.
  • Maintains regular communication with individuals as well as all members of the ISP team.
  • Providing direct care for individuals who require varying levels of physical assistance.
  • Weekly data tracking of goals and objectives.
  • Monthly progress notes for individuals goals and objectives
  • Knowing each individual’s goals and objectives in his or her assigned classroom, as well providing instruction, implementing trials, and recording results of each trial.
  • Develop and implement a classroom schedule, including group and individual activities that our participants can engage in while at Day Program.
  • Assuring goals and objectives are focused on needed areas such as self-care, activities of daily living, communication, social, and motor skills.
  • Maintains electronic case files for caseload (I Centrix) as well as modified physical case records (mini binders).
  • Trai staff in the appropriate work skills for the job position.

Job requirements include:

All CASE MANAGER candidates must be able to successfully pass a CORI, reference, multi-state background, and driving record check.
