Day Habilitation Programs

Supporting Growth, Wellness, and
Community Engagement

Lifeworks offers two vibrant Day Habilitation programs—located in Westwood and West Roxbury—designed to support adults with intellectual and developmental disabilities through personalized, goal-oriented services. Our programs welcome individuals with Autism, Cerebral Palsy, Epilepsy, mobility and sensory impairments, and complex medical needs.

Each participant receives medical oversight and an individualized Day Habilitation Service Plan developed by a team of clinicians and direct support professionals. Lifeworks Day Habilitation programs are funded by the Massachusetts Department of Medical Assistance, accredited by CARF, and maintain a 1:7 staff-to-participant ratio. Personal care assistance, including support with feeding and activities of daily living, is provided.

Westwood Day Habilitation Program

Located at our fully accessible headquarters, the Westwood program is a dynamic environment that blends therapeutic supports with meaningful day-to-day activities. Participants enjoy structured programming that encourages independence, creativity, and social connection.

Health & Wellness:

  • Daily movement and balance groups 
  • Walking club and paved outdoor path 
  • Meditation and yoga 
  • Raised garden beds for therapeutic gardening

Creative Arts:

  • Art and craft classes 
  • Music therapy 
  • Group cooking in a large, accessible kitchen 
  • Interactive tech tools for learning and communication

Community Engagement:

  • Local outings to shops, parks, and museums 
  • Participation in seasonal community clean-ups and school-based partnerships 
  • Special events such as comfort dog visits and holiday celebrations

Therapeutic Supports:

  • Full-time nursing 
  • OT, PT, speech and behavioral consults 
  • Individualized Day Habilitation Service Plans with team-based review

ELIGIBILITY: Participation in Day Habilitation requires MassHealth coverage and physician authorization. To learn more or to explore a potential referral, please contact your service coordinator or reach out to our program teams directly.

Connections – West Roxbury Day Habilitation Program

Connections, based in West Roxbury near the Dedham line, offers a welcoming and supportive environment where participants build essential life skills, explore their interests, and take part in the community. The program is ideal for individuals who thrive in small-group settings and benefit from a well-rounded daily routine.
